Chronic diseases are long-lasting conditions that can be managed at Health Hub Narwee to achieve the best health outcomes. The GP Management Plan is a way to help you manage your chronic disease. You and the doctor will decide on it together, which can include:

  • identifying your healthcare needs
  • setting out what services that they’re going to provide for you (e.g., blood test)
  • connecting with other providers who treat this condition as well so that everyone’s up-to-date about things like medicine changes or lab results
  • and also putting in writing the actions you’ll take

Once your chronic disease management plan is in place, a schedule for regular review will be set by your GP. This meeting provides the opportunity to agree on any changes that may need to be made and ensures you are receiving appropriate care.

Allied health professionals such as physiotherapists or dietitians can be part of your chronic disease management plan.
